symptoms?
7) Is this the fi rst treatment you have sought for this problem? For example,
have you sought help from a temple, an astrologer, ayurveda, allopathy
or anywhere else?
If answer to question 7 is “no,” go to question 8. If “yes” go to question 11.
8) (If previous treatment was diff erent therapy) why did you change
