gradient:ithasbecomevirtuallyatravelers’diseaseincentralEurope.Im-
portedcasesfrequentlycauseminoroutbreaksinfamiliesorschools.Active
immunizationwithaninactivatedHAVvaccineisavailable.
Rhinoviruses
Pathogens.Thegenomicorganizationandreplicationsystemoftherhino-
viruses(1 17 serotypesfoundtodate)generallymatchthoseoftheentero-
viruses,althoughtheydifferinthattheyareacid-sensitiveandslightly
denser.
Pathogenicityandclinicalpicture.Therhinoviruses,thecausativepathogens
ofthecommoncold,infectthemucosaofthenasopharyngealspace(nose
andthroat).Theyremainstrictlylocalizedthereanddonotcausegeneralized
infections.Inrarecases,mainlyinchildren,theyareknowntocausebron-
chitisorbronchopneumoniaaswell.Theclinicalpictureisoftenworsenedby
bacterialsuperinfection.
Diagnosis.Laboratorydiagnosticsareonlyrequiredinspecialcasesofrhino-
virusinfection.Thevirusescanbegrownincellcultures.
Epidemiologyandprevention.Rhinovirusesaretransmitteddirectly,for
examplebycontaminatedhands,andpartlybydropletinfectionaswell.In-
fectivecontactsbetweenhumansappeartoinvolvemechanicalinoculation
(introductionintothenasopharyngealspacewithfingers).Rhinoviruses
occurworldwide,withpronouncedproliferationinthewintermonths.
Thefactthateveryonecomesdownwithcoldsrepeatedlyisexplainedby
theverybriefimmunityconferredbyinfectionandthemanydifferentviral
typesinvolved.Experimentshaveshownthattheinfectionsarealways
exogenous,i.e.,notreactivationsduetocold,wetness,etc.Theonlyconceiv-
ableprophylacticmeasureistoavoidlargegroupsofpeople.
AstrovirusandCalicivirus;HepatitisE
&Astroviruses,measuring 28 – 30 nmandcaliciviruses, 30 – 35 nm,areen-
teritispathogensinsmallchildren.Humanpathogensinthesegroupsinclude
theNorwalkvirusandhepatitisEvirus(HEV).Thelatteroccursepidemically
andendemicallyinAsian,CentralAmerican,andAfricancountries.Itistrans-
mittedbythefecal-oralroute,aboveallviadrinkingwater,andcausesrela-
tivelybenigninfectionsexceptinpregnantwomen.HepatitisEisconsidered
atraveler’sdisease. &
