evidence is it is failing.


THIS IS A book about the modern experience of
mortality—aboutwhat it’sliketobecreatureswhoage
anddie,howmedicinehaschangedtheexperienceand
howithasn’t,whereourideasabouthowtodealwithour
finitudehavegottherealitywrong.AsIpassadecadein
surgicalpracticeandbecomemiddle-agedmyself,Ifind
that neither I nor my patients find our current state
tolerable. But I have also found it unclear what the
answersshouldbe,orevenwhetheranyadequateones
are possible. I have the writer’s and scientist’s faith,
however, that by pulling backthe veil and peering in
close,apersoncanmakesenseofwhatismostconfusing
or strange or disturbing.


Youdon’thavetospendmuchtimewiththeelderlyor
thosewithterminalillnesstoseehowoftenmedicinefails
thepeopleitissupposedtohelp.Thewaningdaysofour
livesaregivenoverto treatmentsthataddleourbrains
andsapourbodiesforasliver’schanceofbenefit.They
are spent in institutions—nursing homes and intensive
careunits—whereregimented,anonymousroutinescutus
off from all the things that matter to us in life. Our
reluctanceto honestlyexaminetheexperienceofaging
and dyinghasincreased theharmweinflictonpeople
and denied them the basic comforts they most need.
