4.12/5/11: Patient presents after auditory examination
revealed a partial sensorineural loss that was distort-
ing his perception of certain frequencies; partially
correctable with amplification. Patient still leans
close to speaker, but in a one-to-one conversation,
his responses demonstrate his ability to correctly
interpret most of what the speaker is saying. He
reports that he still has great difficulty listening in
groups. His wife notes with delight that he seems
“more like his old self” when one or two friends
come to visit. He expresses an interest in learning to
lip-read.—D. Mason, RNCHAPTER 45

Prioritization Questions
1.b.Diabetes: Some men may be candidates for a
penile prosthesis; pharmacologic management of
erectile dysfunction may be indicated.
c. Cardiovascular disease: Teach gradual
resumption of sexual activity, comfortable posi-
tion for affected partner.
d.Loss of body part: Teach acceptance of body
image.
e.Spinal cord injury: Promote stimulation of other
erogenous zones.
f. Mental illness: Provide counseling for depression
g.Sexually transmitted infections: Educate the pub-
lic about the prevention and treatment of STIs.- a.Follicular phase: Days 4 to 14; a number of folli-
cles mature, but only one produces a mature
ovum; at the same time, in the uterus the
endometrium is becoming thick and velvety in
 preparation for the fertilized egg.
 b.Proliferation phase: Ovulation occurs on day 14;
the mature ovum ruptures from the follicle and
is swept into the fallopian tube. If sperm are
present, the ovum is fertilized at this time.
 c. Luteal phase: Days 15 to 28; the empty follicle
fills with a yellow pigment and is then called the
corpus luteum, which produces hormones that
encourage a fertilized egg to grow. If fertilization
does not occur, the corpus luteum disintegrates.
d.Secretory phase: The endometrial lining becomes
thick; in the absence of fertilized egg, the corpus
luteum dies, and the endometrial lining disinte-
grates; menses begins on day 28 as a result of the
 uterus shedding the endometrial lining.
- a.Excitement phase:
Female: The breasts of the woman swell and nip-
ples become erect; vaginal lubricant seeps out of
body; upper two thirds of vagina expand; clitoris
 enlarges and emerges slightly from clitoral hood;
labia enlarge and turn deep rosy red.
Male: Erection of the penis caused by increased
congestion with blood; scrotum noticeably ele-
vates, thickens, and enlarges. The skin of the
 penis and scrotum turns deep reddish-purple;
male nipples may harden and become erect.
 b.Plateau:
Female: The clitoris retracts and disappears under
clitoral hood; intensity is greater than that of
excitement phase but not enough to begin
 orgasm.
Male: Secretions from Cowper’s glands may appear
at the glans of the penis during this phase.
 c. Orgasm:
Female: The orgasm phase begins with a height-
ened feeling of physical pleasure followed by
 overwhelming release and involuntary
contractions of the genitals. Loss of muscular
 control can cause spastic contractions.
Male: Involuntary spasmodic contractions of the
genitals occur in the penis, epididymis, vas
deferens, and rectum; most often accompanied
 by ejaculation.
