Rheumatic Fever

What Went Wrong?

Rheumatic fever (RF) is an inflammatory disease that occurs after an infection

with group Ab-hemolytic streptococcalpharyngitis. The illness is self-limited

and involves

Joints, skin, brain, serous surfaces, and heart.

Cardiac valve damage is the most significant complication.

Signs and Symptoms

Lesions called Aschoff bodies

Carditis involving the endocardium , pericardium, and myocardium

Apical systolic murmur

Polyarthritis

Erythema marginatum, a clear rash often over trunk and proximal portion

of extremities

Subcutaneous nodules

Chorea

Test Results

There is no single definitive laboratory test to diagnose RF.

Clinical and laboratory findings are considered along with evidence of

a recent streptococcal infection.

Treatment

Ten-day course of antibiotic therapy

Salicylates to control the inflammatory process

Nursing Interventions

Promote compliance with the medication regimen.

Support child and family and return to recovery.

Prevent reoccurrence of the illness.
