Exhibit 4-66. Heart rate greater than specified criteria Stage 1 (12 through 49 years)


„ Ideally the HR should be between 60 to 80 percent PMHR during Warm-up for all age
groups.

„ If the heart rate is greater than 80 percent PMHR at any time during Stage 1 (for both
age groups), determine if the SP is OK and able to continue the exam. If the SP is OK,
continue with the test and monitor closely for the remainder of the exam. If the SP
looks exhausted or does not want to continue, go to Recovery.

„ When the data are captured at the end of Stage 1, the ISIS system will display a
message: ‘Heart rate is greater than 80 percent PMHR’. If SP is OK, continue with the
test. If not, go to Recovery.’

„ Click OK.
