Exhibit 6-8. Quality control weekly checks (1)

„ Complete all daily checks. Check ‘Done’ for each item on the weekly checks when
complete. See Exhibit 6-8.

„ Check speed of the treadmill. See Section 2.1.2.1 for calibration procedures for speed
of treadmill. If the speed readout on the treadmill controller exceeds +/- 0.2 mph of
actual speed, repeat the procedure. If the treadmill still exceeds the range, enter the
difference between the belt speed on the controller and the calculated belt speed in the
‘Result’ field. (Example: Difference = 0.8 mph). Enter the speed of the treadmill
controller and the calculated belt speed in the ‘Comment’ field. (Example: Controller
speed = 2.5 mph; Belt speed = 3.3 mph). Report this to the chief technologist. Note
this report in the ‘Comment’ field. (Example: Reported to chief technologist). The
chief technologist or MEC manager will call a service representative.

„ Check grade of the treadmill. See Section 2.1.2.2 for calibration procedures for grade
of treadmill. If the elevation error exceeds +/- 1% grade, repeat the calibration check.
If the elevation error still exceeds +/- 1% grade, enter the elevation error in the
‘Result’ field. (Example: Elevation error = 1.5%). Enter the treadmill grade setting
and the measured grade in the ‘Comment’ section. (Example: Treadmill grade
setting=20%; Measured grade=21.5%). Report this to the chief technologist. Note the
report in the ‘Comment’ section. (Example: Reported to chief technologist.). The
chief technologist will call a service representative.
