If the reason the BMI could not be calculated was due to SP refusal to have weight
and height measured, select ‘Other, specify’ for comments and enter ‘Unable to get ht
and wt information - SP refusal in the ‘Other text’ field.
If the reason the BMI could not be calculated was another reason, enter the
appropriate information in the ‘Other text’ field.
