104 Amaurosis Fugax Amblyopia
specific therapy
■Carotid endarterectomy for ipsilateral localized stenotic lesion (70%
or more)
■Treat or control cardiac arrhythmia or continue warfarin
■General measures: stop cigarette smoking, treat underlying medical
disorders (polycythemia, hypertension, diabetes, hyperlipidemia)
follow-up
■As necessary to control anticoagulants for disturbance of cardiac
rhythm
■Every 3 months to ensure no further TIAs or episodes of amaurosis
fugax
complications and prognosis
■Stroke may occur w/o treatment; see “Transient Ischemic Attacks”

■Loss of visual acuity:
➣More profound when snellen isotypes read in series equaling
crowding phenomenon
➣More profound in recognition acuity than spatial frequency tests
➣More profound in reducing background luminance or w/
neutral-density filter in front of involved eye
Signs & Symptoms
■Afferent pupil may be positive in 1/4 to 1/3 of cases

■Normal w/ usual clinical suspects

■ECG normal
■VEP (pattern) normal or? amplitude
