5.1 Lockout/Tagout (LOTO) Devices
You must provide locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other
hardware for isolating, securing or blocking machines or equipment from their energy sources.
As mentioned in Module 1, lockout/tagout devices hold energy-isolation devices in a safe or "off"
position. Below are a few examples of locks and tags used to lockout or tagout main disconnects, line
valves, and circuit breakers.

5.2 Hardware Requirements
There are specific requirements for LOTO devices to make sure they fulfill the purpose for which they
are used. They must ensure authorized employees are safe from injury while working on equipment
and machinery.
LOTO devices must prevent other workers from accidentally energizing equipment or machinery while
authorized employees are exposed to the various hazards of unexpected startup or shutdown.
To do that, LOTO devices must be substantial enough to prevent removal without the use of excessive
force or unusual techniques such as with the use of bolt cutters or other metal cutting tools.
