●● The arch of the azygos vein as it crosses the esophagus is
divided and transfixed. The right bronchial artery, which can
be quite sizable, runs just beneath the vein. It can be preserved
if desired. The superior mediastinal segment of the esophagus
can be mobilized close to its wall. The mediastinal pleura is
opened on the back of the trachea. The esophagus is freed
from the back of the trachea anteriorly and posteriorly from
the spine; it can be slung with a tape to ease later dissection.
Dissection is carried out near the apex.
