supporting the critical need for interprofessional and interdisciplinary collaboration,
are to be discussed. Finally, nurse- led clinics that provide a setting for DNP and IPE,
practice, and research are to be described. Nurse- led clinics provide a learning collabo-
ratory where students partner with communities design settings and systems where
children, families, and adults learn, live, work, play, and pray (Novak, 2015). Over
the past 50 years, academic nursing centers have been developing, implementing, and
evaluating alternatives to the failing, mismanaged traditional U.S. health care delivery
system. Most of these efforts have lasted only one to three years due to dependence on
a single grant for funding support. To address this problem, a model of sustainability,
or a “mosaic of support” was created providing a framework for the continued suc-
cess of a replicable nurse- managed health home model. The model emphasizes effec-
tive communication, a business plan, and a broad portfolio of sources of support (see
Figure 17.1 ). DNP students and their interprofessional partners learn how to develop
and sustain an integrated model of learning, discovery, and engagement with key
components related to communication, collaboration, business principles, and diver-
sification of funding streams. Interprofessional students and faculty meet monthly to
innovate and sustain what they have developed in partnership with communities who
have clearly identified their needs. The model reflects the triple aim at one third the
cost of traditional medical models of the last and current century. DNP and other inter-
professional students must learn to collaborate and communicate effectively in practice
