take-off from Juhu during a
coastal surveillance mission and
made a very hard emergency
landing at 1448hrs on an uneven,
rocky area near Alibaug, 6nm

The helicopter’s undercarriage

the underside of the fuselage and

were rescued and flown back to
the Naval Hospital INHS Asvini at
Mumbai. The co-pilot had been
struck by one of the main rotors
on exiting the helicopter and died
from her injuries 17 days later.
The Chetak had been en route
to the Mumbai High North Field,
where it had been scheduled
to land at around 1100hrs.

N/U: USAF/Alaska ANG/176th
Wing/21st RQS
T: HH-60G Pave Hawk
At approximately 1845hrs GMT,
this helicopter crashed 18 miles
(29km) southeast of al-Qaim in
western Anbar Province, Iraq,
killing all seven personnel on
board. US officials said the
crash did not appear to be the

result of enemy activity and it
may have struck a power line
shortly after take-off from the US
logistics base and supply hub at
al-Qaim for a routine transport
mission between Iraq and Syria.

D: Mar 15
N: USAF/433rd AW
T: C-5M Super Galaxy
This aircraft was forced to make
an emergency landing at Joint
Base San Antonio-Lackland,
Texas, with the nose undercarriage
retracted, following a nose gear
malfunction while returning to

base after a training flight. None
of the 11 crew members were
injured. The aircraft came to rest
on its nose after skidding about
three-quarters of the way down
the 11,500ft (3,505m) runway.
Damage to the aircraft is being
assessed. Although this is the
first incident of this kind for the
wing, in July last year Air Mobility
Command ordered a fleet-wide
maintenance review of its 56
Galaxies after two undercarriage
malfunctions. This was preceded
a day earlier by a temporary
grounding of the 18 C-5s based at

Dover Air Force Base, Delaware,
following two nose undercarriage
malfunctions of its aircraft within
a 60-day period, both at Naval
Station Rota, Spain. The ball-
screw drive assembly was found
to have caused problems with
nose undercarriage extension
and retraction. All of these
components have since been
replaced fleet-wide, including
those on the aircraft involved
in the latest incident, but
initial investigation suggests
this is an isolated event.

ejected, but his fate is unconfirmed.
