Her tongue was Pale and tending to be Peeled and her pulse was slightly Floating-Empty.
Diagnosis The pain in the fingers is clearly due to invasion of Cold and Dampness.
It has now turned into chronic Painful Obstruction Syndrome with
stagnation of Phlegm in the joints (as evidenced by the bone
deformities). Initially, this occurred against a background of Blood
deficiency which then began to turn into Yin deficiency (of the
Kidneys). The Blood deficiency is apparent from the Pale tongue, whilst
the beginning of Yin deficiency is reflected in the falling hair, weak
knees and Floating-Empty pulse. This case shows clearly how one can
often see in practice borderline cases between Blood and Yin deficiency
which happen more frequently in women.
Treatment principle She was treated with acupuncture and herbs and the treatment was
aimed at removing obstruction from the channel, expelling Cold and
Dampness, nourishing Blood and Yin.
Acupuncture The main points used were:
L.I.-3 Sanjian and Baxie extra points to remove Cold-Dampness from
the fingers.
T.B.-5 Waiguan to expel Wind.
ST-36 Zusanli and SP-6 Sanyinjiao to nourish Blood.
KI-3 Taixi and BL-23 Shenshu to tonify the Kidneys.
Herbal treatment The prescription used was a variation of Qin Jiao Si Wu Tang Gentiana
macrophylla Four Substances Decoction:
Dang Gui Radix Angelicae sinensis 6 g
Sheng Di Huang Radix Rehmanniae glutinosae 6 g
Bai Shao Radix Paeoniae albae 6 g
Chuan Xiong Radix Ligustici wallichii 6 g
Qin Jiao Radix Gentianae macrophyllae 6 g
Yi Yi Ren Semen Coicis lachryma jobi 6 g
Can Sha Excrementum Bombycis mori 6 g
Gan Cao Radix Glycyrrhizae uralensis 3 g
Ban Xia Rhizoma Pinelliae ternatae 6 g
Sang Ji Sheng Ramus Loranthi 4 g
Explanation Dang Gui, Sheng Di Huang, Bai Shao and Chuan Xiong are a modified
version of the Four Substances Decoction and will nourish both Blood
and Yin since Sheng Di Huang is used instead of Shu Di Huang.
