Empty-Heat. Empty-Heat pushes blood out of the vessels

(c) Stasis of Blood in the vessels may also cause bleeding.

The colour of the urine will vary according to the predominant condition. It will be bright red if
Spleen-Qi deficiency predominates, dark red if there is Empty-Heat and dark with clots or brown
if there is stasis of Blood.


Bearing in mind the above three factors, the principle of treatment in cases of haematuria is to
tonify Qi, nourish Yin, cool Blood and eliminate stasis. To tonify Kidney-Qi and Kidney-Yin
one can use Shan Zhu Yu Fructus Corni officinalis, Du Zhong Cortex Eucommiae ulmoidis, Shu
Di Huang Radix Rehmanniae glutinosae praeparata, Gou Qi Zi Fructus Lycii chinensis, Nu
Zhen Zi Fructus Ligustri lucidi, or Xu Duan Radix Dipsaci. If there is a pronounced deficiency
of Spleen-Qi Huang Qi Radix Astragali membranacei can be used. In cases of deficiency of Yin
with Empty-Heat affecting the Blood one can select Sheng Di Huang Radix Rehmanniae
glutinosae, Mu Dan Pi Cortex Moutan radicis, Bai Mao Gen Rhizoma Imperatae cylindricae,
Qian Cao Gen Radix Rubiae cordifoliae or Han Lian Cao Herba Ecliptae prostratae. If Heat
signs are pronounced add Huang Bo Cortex Phellodendri and Zhi Mu Radix Anemarrhenae
asphodeloidis. In cases of stasis of Blood add Yi Mu Cao Herba Leonori heterophylli, San Qi
Radix Notoginseng or Tao Ren Semen Persicae.


Treatment


The treatment of the Manifestation is based on three main methods: sweating, promoting
urination and purging. The treatment of the Root is based on tonifying.


Sweating is used for only one type of "Yang oedema", i.e. acute nephritis characterized by
invasion of the Lungs by Wind-Water. In such a case, sweating is used because it releases the
Exterior and expels Wind, of which Wind-Water is a type.


Promoting urination is used for chronic nephritis with oedema and Dampness. Within this
method of treatment it is important to distinguish Excess from Deficiency and Cold from Heat.



  1. Excess pattern of Dampness:


(a)Cold Dampness: warm Yang and promote urination
(b)Hot Dampness: clear Heat and promote urination


  1. Deficiency pattern: warm and tonify Spleenand Kidneys.


The purging method is used only in very severe cases of oedema when the above methods of
treatment are unsuccessful. For light purging, one can use large doses of Bing Lang Semen
Arecae catechu (30 g) and Yu Li Ren Semen Pruni (24 g). The purging method should not be
