Exhibit 4-79. Problems with data acquisition - electrodes, BP cuff, and treadmill controller

„ Problem with electrodes - Causes of problems with electrodes include inadequate
skin preparation, chest hair, faulty electrodes, and a large chest or abdomen.

„ Problem with BP cuff - The SP may have a large arm that makes the positioning of
the cuff difficult. If the cuff is not attached snugly to the arm, it may slip during the
exam. In addition, if the SP is flexing when the cuff is applied, the cuff may slip out of
position when the arm is relaxed. Bending or flexing the arm may also cause some
problems. The SP should be encouraged to let his or her arm hang in a relaxed manner
and should avoid extreme flexing although they do not have to hold their arms in a
stiff uncomfortable position.

„ Problem with treadmill controller - If the buttons on the treadmill are inadvertently
pushed in error, the test may be restarted if this occurs before the first 15 seconds in
Stage 1. If this happens after warm-up when changing protocols or going to recovery,
you have the option of going to the next set of protocols on the controller.

„ Check all data acquisition problems that apply. Be very careful that the selection you
make accurately describes the problem you had.
