  1. Would you say that you participated regularly in recreation or work requiring
    modest physical activity?


OR


  1. Would you say that you participated regularly in heavy physical exercise?


If the SP needs further clarification on the meaning of modest versus heavy, use the
following examples:

Modest physical activities include: walking, calisthenics, bowling, weight lifting,
yard work.

Heavy physical activities include: jogging, swimming, cycling, rowing, tennis, and
basketball.

Enter 1, 2, or 3 in the system.

Second Level of Questions for Little or No Regular Activity


„ If the response to the first set of questions is 1 (little or no regular activity), the system
will go to a set of questions to further classify the SP’s physical activity level. A PAR
code will then be assigned based on the response and be used to determine predicted
VO 2 max.


„ Read both questions before accepting a response.



  1. Would you say that you avoid walking or exertion, for example always use
    the elevator, drive whenever possible instead of walking?


OR


  1. Would you say that you walk for pleasure, routinely use stairs, occasionally
    exercise sufficiently to cause heavy breathing or perspiration?


„ A PAR code will then be assigned as outlined below:



  • If response is 1 (avoid walking), the PAR Code will be assigned as 0.

  • If response is 2 (walk for pleasure), the PAR Code will be assigned as 1.

physical activity), the system will go to a set of questions to further classify the SP’s
