statement of QC. Retesting required for CHOC, selective media for Neisseria &Campylobacter.Must be reviewed & signed annually by lab director. Changes must be approved & signed by lab director.
Obsolete procedures must be removed & retained for 2 years.Record all QC results & document any corrective action. Forms should be reviewed & initialed by supervisor
monthly. Maintain for at least 2 years.Verification on hiring & annually.Test unknowns exactly as patient specimens. Must maintain an average score of 80%. Document
corrective action.
