1406 Tapeworm Infections Tardive Dyskinesia

■Hymenolepis nana:
➣Praziquantel single dose
Side Effects & Complications
■All infections: occasional mild nausea, pain, occasional dizziness.
Theoretical risk of auto-infection withT solium, leading to cysticer-
cosis.
■Contraindications to treatment: absolute: allergy to medication.
■Contraindications to treatment: relative: none.

follow-up
During Treatment
■T solium: treat any nausea. Vomiting should be avoided.

Routine
■T saginata: If no segments or proglottids seen at 3 months, patient
cured.
■T solium: check stools at 2–3 months. If no eggs and no segments or
proglottids, patient cured.
■Diphyllobothrium: check stool O&P at 3 months.
■Hymenolepis nana: check stool O&P at one month.
complications and prognosis
■Prognosis is excellent regarding elimination of worms. Patients with
T soliummay have cysticercosis (see cysticercosis).

history & physical
■Typically develops after long-term treatment w/ dopamine receptor
antagonists (may occur w/ both typical & atypical neuroleptics)
■Occurs in 15–30% of pts on long-term neuroleptic treatment
■May develop during treatment or just after discontinuing treatment
■Higher risk in women, advanced age or prolonged exposure
■Higher risk in pts w/ diabetes, HIV or depression
■Movement can be suppressed voluntarily
■Choreoathetoid movement especially conspicuous about the lower
face (eg, lip smacking, puckering, chewing movements, tongue twist-
ing)
■Limb involvement usually symmetric
