  1. Neoadjuvant CRT (CROSS regimen):
    RT dose fractionation: 41.4 Gy in 23 fractions of 1.8 Gy, five
    fractions per week.
    Chemotherapy: weekly paclitaxel (50 mg/m^2 ) and carboplatin
    (AUC = 2) concurrent with RT.
    The whole course of RT lasted for 5 weeks with total five cycles
    of weekly chemotherapy.
    Alternative regime: cisplatin (100 mg/m^2 ) day 1 and 5FU
    (1000 mg/m^2 /day) for 4 days on week 1 and 5 concur-
    rent RT (50.4 Gy/28 fractions over 5.6 weeks).

  2. Definitive CRT (RTOG 8501):
    RT dose fractionation: 50 Gy in 25 fractions over 5 weeks.
    Chemotherapy: cisplatin 75 mg/m^2 and 5FU 1000 mg/m^2 /
    day continuous infusion for 4 days, two cycles concurrent
    with RT and two additional cycles after completing RT.
