femoralamputationsduetocirculatoryproblems.Chemoprophylaxisaimed
atpreventingapostsurgicalinfectionshouldbeginafewhoursbeforethe
operationandneverbecontinuedforlongerthan 24 – 72 hours.
Immunomodulators.......................................
Despitethegenerallygoodefficacyofanti-infectiveagents,therapeuticsuc-
cesscannotbeguaranteed.Completeeliminationofbacterialpathogensalso
requiresafunctioningimmunedefensesystem.Inviewofthefactthatthe
numberofpatientswithsevereimmunodeficienciesisontherise,immuno-
modulatorsareusedasasupportiveadjuncttospecificantibiotictherapyin
suchpatients.Manyofthese“cytokines”(seep.77ff.)producedbythecellsof
theimmunesystemcannowbeproducedas“recombinantproteins.”Mye-
lopoieticgrowthfactorshavenowbeensuccessfullyusedinpatientssuffer-
ingfromneutropenia.Additionalimmunomodulatorsarealsoavailable,e.g.,
interferongamma(IFNc)andinterleukin 2 (IL-2).

&Infectionscanbediagnosedeitherdirectlybydetectionofthepathogenor
componentsthereoforindirectlybyantibodydetectionmethods.Thereli-
abilityoflaboratoryresultsischaracterizedbythetermssensitivityandspec-
ificity,theirvalueismeasuredintermsofpositivetonegativepredictive
value.Thesepredictivevaluesdependtoagreatextentonprevalence.Indi-
rectlaboratorydiagnosis,correctmaterialsamplingandadequatetransport
precautionsareanabsolutenecessity.Theclassicmethodsofdirectlabora-
torydiagnosisincludemicroscopyandculturing.Identificationofpathogens
is based onmorphological, physiological, andchemical characteristics.
Amongthelatter,theimportanceofdetectionofpathogen-specificnu-
cleotidesequencesisconstantlyincreasing.Developmentofsensitive
testsystemshasmadedirectdetectionofpathogencomponentsintestma-
terialspossibleinsomecases.Themolecularbiologicalmethodsusedareap-
pliedwithorwithoutamplificationofthesequencesoughtasthecasewar-
rants.Directdetectioncanalsoemploypolyclonalormonoclonalantibodies
todetectandidentifyantigens. &
