Diagnosis


Theprimaryconcernhereisidentificationofthepathogen.
&Microscopy.Nativepreparation:brieflyheatmaterialundercoverslip
with 10 %KOH.Stainedpreparation:stainwithmethyleneblue,lactophenol
blue,periodicacid-Schiff(PAS),ink,etc.
&Culturing.Thisispossibleonuniversalandselectivemediums.Sabour-
auddextroseagarcancontainselectiveagents(e.g.,chloramphenicolandcy-
cloheximide),thismediumhasanacidpHof5.6.Themainidentifyingstruc-
turesaremorphological,inparticulartheasexualand,ifpresent,sexualre-
productivestructures.Biochemicaltestsareusedmainlytoidentifyyeasts
andaregenerallynotasimportantinmycologyastheyareinbacteriology.
&Serology.Bytheidentificationofantibodiestospecialfungalantigensin
patient’sserum.TheInterpretationofserologicalfindingsisquitedifficultin
fungalinfections.
&Antigendetection.Byfindingofspecificantigensinthediagnosticma-
terialbydirectmeansusingknownantibodies,possibleinsomefungalin-
fections(e.g.,cryptococcosis).
&Cutaneoustest.Cutaneous(allergy)testswithspecificfungalantigens
canbeusefulindiagnosinganumberoffungalinfections.
&Nucleicaciddetection.Combinedwithamplification,suchtestsareuse-
fulforrapiddetectionofmycoticdiseasesinimmunocompromisedpatients.

Therapy


Alimitednumberofanti-infectiveagentsareavailableforspecifictreatment
offungalinfections:
&Polyenes.Theseagentsbindtomembranesterolsanddestroythemem-
branestructure:
— AmphotericinB.UsedInsystemicmycoses.Fungicidalactivitywithfre-
quentsideeffects.Thereareconventionalgalenicformand(new)various
lipidforms.
— Nystatin,natamycin.Onlyfortopicaluseinmucosalmycoses.
