Study Guide for Fundamentals of Nursing The Art and Science of Nursing Care

(Barry) #1
Evaluative Statement: 3/22/11: Expected outcome
partially met—on two occasions, Toni remained in
control of her breathing during her mother’s visits.
On at least one occasion, she hyperventilated.
—K. O’Leary, RN
3.Patient strengths: The patient’s strengths still need
to be identified; mother seems to be gaining an
appreciation of her needs.
Personal strengths: Good understanding of the
effects of stress and experienced in helping patients
replace maladaptive coping strategies with adaptive
strategies.
4.3/22/11: This morning, Toni began talking about
the difficult situation at home. When asked about
her relationship with her mother, she began
gulping for air but “caught herself” and quickly
reestablished control of her breathing, consciously
decreasing her rate and depth. Whereas she has
shown no signs of hyperventilation on two of her
mother’s last visits, she had one episode in which
she hyperventilated and totally “lost control” and
needed sedation. She stated, she feels like she is
making progress but still has a long way to go
before she will feel comfortable at home and in
control of simple, everyday things like breathing.
—K. O’Leary, RN
