Skywest Airlines flight OO5869, on behalf of United Express

from Aspen (CO) to Denver (CO), landed on Denver’s runway

34R and was vacating the runway when flames became visible

from the ground from the left hand engine. The CRJ stopped

on taxiway F(oxtrot) near the northern end of F, with the pas-

sengers rapidly disembarking the aircraft via aircraft stairs.

Emergency services sprayed the left hand engine and ground

and quickly extinguished the fire. The final damage is to be

determined by the NTSB.

Kids Flight 1, an air ambulance helicopter service operated by

Air Methods for the St. Louis Children’s Hospital, experienced

an emergency autorotation and subsequent hard landing in

farmland north-east of Perryville in Perry County (MO). It

came to rest on its side, sustaining substantial damage, and

the five occupants (pilot, three medical crew and a five-year

old patient) survived but were taken to hospital with unde-

termined injuries. During the flight the pilot reported that

when the flight was about five miles north of Perryville, the

chopper “experienced a sharp change in attitude yawing to

the left with a hard-upward bump,” followed by a change in

the engine noise. He observed the N1 gauges (an engine indi-

cation parameter) both indicating below forty per cent and

decreasing. The no. 1 engine low warning light, the no. 1 gen-

erator light, and the battery discharge warning lights were

illuminated. He stated that “Suddenly the aircraft pitched

nose up and rolled to the right. I could hear the rotor begin

to deteriorate.” He entered an autorotation by applying

right forward cyclic and lowering the collective to full down.

During the autorotative descent, he saw power lines and a

ditch which required him to change his flight path to land on

the far side of the ditch. He flared the helicopter about 100ft

and the rotor RPM began to decay rapidly. He attempted to

level the helicopter “as it began to fall through.” The helicop-

ter landed right skid low and the helicopter skidded for about

100ft. The main rotor blades hit the ground as the helicopter

rolled onto its right side. Once the helicopter came to a rest,

he pulled the power levers to the stop position and ordered

an evacuation.

Shortly after take-off from Alpine-Casparis Municipal Airport

(TX), while on a cargo flight to Midland (TX) on behalf of UPS,

the pilot of this Martinaire Grand Caravan encountered prob-

lems with the engine and elected to return for a safe landing.

Shortly after it struck power cables near Highway 18 and

crashed in a field. The pilot was able to walk away and was

uninjured while the aircraft was damaged beyond repair.

04jul17 Dhruv w/o

ages from Itanagar to Naharlgar) near Sagalee in Papum Pare

This Gulfstream III, operated by the Venezuelan govern-

ment agency Servicio Coordinado de Transporte Aéreo del

Ejecutivo Nacional, crashed into the sea off Macanao, Vene-

zuela. The maritime SAR agency ONSA Venezuela stated that,

according to preliminary information, the flight reported a

hydraulic failure and control difficulties while forty miles

from Porlamar-del Caribe Santiago Mariño International

Airport. In less than three minutes the crew reported being

at 3,000ft, preparing to ditch.

The following day two bodies were retrieved from the water.

Seven other occupants were still missing, though believed

Papillon Grand Canyon Helicopters had to send another chopper to pick up its pilot and his four passengers, after the chopper struck power lines

and had to make a forced landing near the Hoover Dam (AZ), during which it sustained substantial damage. N151GC is seen here basking in the

Arizona sunlight on 22 October 2016 when Walter Heukensfeld took this picture at Grand Canyon-National Park.
