I have the greatest admiration for W. W. Norton, which remains a very
special independent publisher. My friend Donald Lamm, former chairman
of the board and former president of Norton, has always embodied the best
in publishing. He and Steve Forman, a wonderful editor and valued friend,
first proposed this project to me. Alan Cameron in London’s Norton office
has long provided support. Sandy Lifland’s deft and patient work as develop¬
mental editor contributed greatly to the first edition. Sarah England, then
Steve’s assistant and now an editor at Norton, was extremely helpful through¬
out the preparation of the second edition, and Kate Feighery and JoAnn
