peak testing days in order to observe as many CV tests as possible. The consultants will complete the
Technician Competency Evaluation form (See Section 6.5.1) during each test and review the completed
form with the technologist. Any necessary improvements that need to be made by the technologist will be
documented on the form, and the technologist will sign the form after reading it. Technologists who are
observed having CV testing problems during field visits will be observed more frequently than other
technologists. A secondary activity during each site visit will be to monitor the physician’s examination
to ensure that the CV exclusion questions are being asked according to the script.
Nonexpert quality control visits are conducted twice a year for each team. Westat
development staff for this component will conduct these visits to monitor procedures and to conduct re-
training for changes in procedures if necessary. A Site Visit Report form is completed by the consultants
and development staff following each field visit. The Site Visit Report form is used to identify, document,
and follow up on issues or problems noted during observational field visits. In contrast to the Technician
Competency Evaluation form, the Site Visit Report form is used to give an overall assessment of the
status of the component and team performance. The intent is to report general problems and issues rather
than problems and issues regarding the specific performance of specific technologists. The latter is
reported on the Technician Competency Evaluation form.
6.5.1 Technician Competency Evaluation Form
This form is used by consultants and trainers to evaluate health technologists conducting the
exam. Items are checked as Pass/Fail. If the technologist does not pass an item, the observer will discuss
this with the technologist, offer suggestions for improvement, and re-evaluate performance in a
subsequent exam later in that visit or at the next visit. See template form in Exhibit 6-16.
